What gearing are you normally in at the start of a TT with a guy holding you for the start. i have crashed a few times out of the gate. when does the guy let go? should you stand and pedal. or stay in the saddle? what gear? please help!

Have a gear that you are able to pedal. He'll let you go when you're supposed to go. Not before, nor later. Just start pedaling when the clock say zero, and you schould be allright.
As I'm used to riding on a pretty high cadence, I usually start of on a pretty light gear, beeing something like 52x18
